Need a particular form, but don’t know where to find it? Student forms for the Registrar’s Office are listed below.

Please read all instructions carefully before completing and submitting any form. Forms that are incomplete, missing required documentation, or submitted to the incorrect office will result in a delay in processing.

Submitting the Form

Once you have completed the form, submit the completed form along with any applicable documentation using one of the following methods:

  • Email: Must be sent from your official York Tech email address to arecords@yorktech.edu.
  • Campus Drop Box: Located on main campus at the entrance of Building J.
  • In Person: At the Registrar’s Office during business hours in Building J.
