FERPA Consent Form
The Family Educational Rights and Privacy Act (FERPA) protects the privacy of student education records. In compliance with the FERPA regulations, only the student to whom the education records pertain to may authorize the release of such records. Accordingly, the FERPA Consent Form must be completed, signed, and submitted by the student. It may not be completed or submitted by a parent, guardian, or any other individual on the student’s behalf.
Below, we have provided common questions related to the FERPA Consent Form:
Step 1: Log into your Navigate account
Step 2: Click on Resources - Places
Step 3: Select the Forms folder and click on “Complete FERPA form”
Step 4: Click on the link displayed to open a new window
Step 5: Log into YTC eForms using your Navigate User Id and Password
Step 6: Under the Document Library, click the “FERPA Consent Form” form and complete the fields contained within the form
Step 7: Once fields are completed, click send
In compliance with FERPA, York Technical College verifies directory information as requested by third parties. Directory information consists of the following information:
- Student Name
- Address
- E-mail Address
- Telephone Number
- Dates of Attendance
- Full-Time/Part-Time Status
- Program of Study
- Anticipated Graduation Date
- Awards
- Honors
- Degree, Diploma, or Certificate Conferred
- The student and any outside party who has the student’s written consent.
- School officials (as defined by York Technical College) who have a legitimate educational interest.
- Parents of a dependent student as defined by the Internal Revenue Code.
- A person in response to a lawfully issued subpoena or court order, as long as the college makes a reasonable attempt to notify the student first.
York Technical College adheres to the rules and regulations of the Family Educational Rights and Privacy Act (FERPA) to protect the privacy of student information; however, students also have a responsibility to protect their information. Students access college computing systems such as Desire2Learn and Self Service via login information (username and password). Students should NOT share their login information. Releasing college system login information to others may be considered a violation of the Student Code of Conduct.
